Key Findings

  • Two-touch reminders generally outperform a single reminder.
  • SMS tends to beat email for confirmation rates.
  • Reminders that name the provider and visit reason perform better than generic ones.

Why No-Shows Matter More Than They Used To

A single no-show costs a primary care practice a meaningful amount in lost revenue and downstream rescheduling. With margins compressing, even a small improvement in attendance can fund real operational gains over a year.

What Tends to Work

Across the industry, cadence and channel consistently matter, and personalization compounds the effect. A practical starting point:

  • Send the first reminder a few days before the visit
  • Send a second reminder ~24 hours before, on a different channel
  • Default to SMS for confirmation; reserve email for richer details
  • Name the provider and the visit reason — generic reminders underperform

What Tends Not to Work

Adding a third or fourth reminder often produces diminishing returns and increases opt-outs. For chronic no-show patients, a personal phone call from front-desk staff usually outperforms any automated cadence.
